It’s usually one of the first questions patients ask when they sit down to talk about Invisalign, and it’s a fair one. Nobody wants to commit to a treatment without some sense of how long they’ll be wearing aligners.
The honest answer is: it depends on what your teeth are actually doing. Invisalign timelines vary more than people expect, and the number quoted in marketing material rarely matches the number a specific patient ends up with. What follows is a realistic breakdown of the ranges, the factors that move you between them, and why the finish line sometimes shifts once you’re already partway through.
The Three Broad Ranges
Every case is planned individually, but most fall into one of three general bands.
Mild cases — roughly 6 to 12 months. This covers minor crowding, small gaps, or slight rotation of a few teeth. If your bite is otherwise healthy and the movement needed is limited to the front teeth, treatment tends to sit at the shorter end of the Invisalign spectrum.
Moderate cases — roughly 12 to 18 months. This is where a meaningful proportion of adult patients land. It might involve broader crowding, larger gaps, or some correction to how the back teeth meet. More teeth are moving, and often through a greater distance, which naturally extends the plan.
Complex cases — up to 24 months, occasionally longer. Significant crowding, bite corrections such as overbite or crossbite, or teeth that need to rotate substantially all add time. These cases usually involve more aligner sets and closer monitoring along the way.
These ranges are a starting point, not a promise. Your dentist will only be able to give you a specific estimate after looking at your teeth, taking a digital scan, and reviewing the 3D treatment plan.
What Actually Determines Where You Land
A few factors do most of the work in deciding which band you fall into.
Compliance
Invisalign aligners are designed to be worn 20 to 22 hours a day. That’s not a rounded-up marketing figure — it’s the amount of time needed for each aligner to do its job before you move to the next one in the series.
Take them out more than that — for extra meals, for long social occasions, for the days you simply forget — and the teeth don’t move as planned. Some patients find their treatment stretches well past the original estimate for exactly this reason. It’s the single biggest variable in the entire process, and it’s the one within a patient’s own control.
Case Complexity
The more teeth need to move, and the further they need to travel, the more aligner sets are required. A patient with a single crowded tooth and a patient with generalised crowding across both arches are simply not on the same timeline, even if both are technically “Invisalign patients.”
Age and Biology
Tooth movement relies on biological processes in the bone and ligament supporting each tooth, and those processes don’t run at the same speed for everyone. This isn’t a reason to rule Invisalign out at any particular age — it’s simply one more variable your dentist accounts for when estimating a timeline.
What Are Refinement Trays, and Why Do They Extend Treatment?
This is the part that catches people off guard, mostly because it isn’t always mentioned upfront: most Invisalign cases don’t end with the last tray in the original set.
When your treatment plan is built using Invisalign’s ClinCheck software, it maps out a predicted path of movement from your starting position to your target position. It’s a genuinely useful planning tool — but it’s a prediction, not a guarantee. Teeth don’t always move in a perfectly straight line to where the software expects.
When you finish your original series of aligners, your dentist checks the result against the plan. If everything has tracked as expected, you move to retainers. If some teeth haven’t quite reached their target position — which is common, not a sign anything has gone wrong — a new scan is taken and a further set of aligners, called refinement trays, is made to finish the movement.
Refinement trays are a normal part of Invisalign treatment, not a failure of the original plan. But they do add real time — typically another 8 to 16 weeks, sometimes more for a second round. If your dentist mentions refinements partway through your treatment, it’s worth understanding that as a routine adjustment rather than a setback.
Managing Expectations From the Start
Invisalign is often marketed on discretion and convenience, and it delivers on both. What it doesn’t deliver is an instant result. Even the shortest cases take months, and the aligners need to be worn according to the plan for that timeline to hold.
